This online maintenance form is to be used to report any maintenance requests and or issues that need to be addressed on our Holy Family campus. Maintenance requests will be completed in the order received and based on level of urgency by our maintenance staff. Thank you!

Here's how to properly submit a request:
STEP 1
Fill out the form below providing as much detail as possible regarding the item or area needing service.
STEP 2
Send a photo or photos of the item/area needing service to [email protected]. In the subject line, please include your first name, last name, and the location or item for the work order so our team can identify which work order request it belongs to.
